refused connection from x.x.x.x but iptables are flushed?

So I have a machine internally, I can ssh into it no problem. But when I try to ssh from outside it failed. Check the logs and it said refused connection from x.x.x.x (x.x.x.x) I check the iptables and it's flushed. How can that be? Without ip tables it should be accepting connection from anywhere no?
